Complete the Colorado State University enrollment
form. Include a payment of $150
for each 2-credit workshop or $225 for each 3-credit course
you select. Checks and money orders should be made
payable to Colorado State University.

Please provide the name of the workshop or course, the semester
for which you are enrolling, and have your credit card
and
personal information available.
You cannot receive credit for the
same workshop or course twice.
As each graduate level credit represents 15 class hours
plus homework time, you are limited to 5 credits per semester
(two 2-credit workshops, or one 3-credit course and one 2-credit
workshop). All
Annenberg Media educational theory and issues workshops through
Colorado State University are offered under the course number
of ED591 with different areas signified by sub-course letters
(e.g. Math: What's the Big Idea?
is "ED591FV: Annenberg Media Workshop in Mathematics
Instruction," or Science in Focus: Energy is "ED591EV:
Annenberg Media Workshop in Science Instruction").
Please note that changing the semester for which you receive
credit, after you have registered, entails a $25 processing
fee. This request must be received prior to the last date
of enrollment of the original semester for which you are registered.
To avoid unnecessary charges, be sure to select the
semester most appropriate for your schedule, and give yourself
ample time to view the video programs and to complete the
coursework. Incompletes will only be granted for
extreme and unusual circumstances. Please refer to the Dates
and Deadlines Table in order to make your choice:
