Step 1: Facilitator's Info
This is a multi-page form to allow you, the workshop facilitator, to send us information about yourself and the other workshop participants in your group, so that we can respond to any inquiries from your group and so that we can send you official Harvard-Smithsonian Certificates of Participation at the conclusion of the workshop. First, fill out your own information in the form on this page, then click the "Next Step" button to begin adding the other workshop attendees. Make sure you've gathered all the necessary information -- the name, phone number, e-mail address, and school and (optionally) home addresses -- for each attendee before you begin. If you led
more than one workshop or course, please submit a separate form for each.
Please note: Certificates are not available for New
American Schools or Critical Issues in School Reform. Also, because there is no way to validate the number of sessions attended or the work that is completed by individuals working alone, as of April 15, 2004 certificates will only be granted to those who are working in a group.
